Click on the “
create a monthly report
” button for the contractor permit shown.
This will take you to the report form.
Create Monthly Report
If you are reporting “
zero
” activity – you had no work on mine sites for the month, then click on the “
no activity
” button.

If you did have work activity on a mine site, then click on the “
create a mine site report button
”, pick the company for which you provided services, then select the WV Mine Permit from the list of permits for that company, and enter your information (injuries, employees and hours.)

Select a Mining Company
Select A Mine Permit Number
Enter Injury, Employee, and Hour Information
Click on the
save button
– this will take you back to the report form – if you have additional mine permits that you need to report activity on, click on the “create mine site report button again and complete the information, save again. Repeat this process until you have reported on all of the WV Mine Permits your company had activity.
Save the Mine Site Report
Mine Site Report Error
Some errors will prevent you from saving the mine report, and will force you to return to the mine report to enter the correct values. For example, the following values - 3 injuries, 20 employees, and 0 total hours - is obviously a mistake.
The following message will be displayed when an attempt is made to save the mine site record.
Click on "Go Back to your Report" to return to the mine site report, enter the correct values, and save the report again.
Mine Site Report Appears in Monthly Report
All mine site reports that are saved and accepted are displayed on the Monthly Report. On returning to the Monthly Report page after saving a mine site report, the Monthly Report will be updated automatically to display the current list of associated mine site reports. The mine site reports will appear in the center of the Monthly Report between the two dark blue bars. The mine site reports are grouped by mine site company. The WV Permit Number/Location for the mine site will be displayed along with the injury, employees, and hours values.
To View, Edit, or Delete an Existing Mine Site Report
Repeat Process for All Mine Sites
Once you have completed all of the WV Mine Permit reports, then click on the
Submit Report Button
.

If you have more than one Contractor Permit to report for, you can then click on the
select a new contractor
button and go through the process for it.
Once the report has been processed you will receive a verification via email.
