Online Independent Contractor Reporting

The West Virginia Office of Miners' Health, Safety and Training is responsible for the enforcement of the West Virginia Mine Safety Laws and Regulations (Chapter 22A Code of WV). As part of that responsibility, independent contractor reports are filed with this office
Reports are filed monthly by independent contractors holding valid WV Office of Miners' Health Safety and Training permits.  The law gives them 31 days from the end of the reporting month to file.  The reports have to be posted in our office and data compiled and loaded to the website.
To facilitate the filing process, an independent contractor may be able to file reports on line.
If you have been approved to file reports on line, please login by clicking on the button at the bottom right of this page.
If you would like to submit a request to enter reports on line, please click on the button below to make that request.
If you file your reports on line, please do NOT also mail or fax a paper copy to MHST
